Polk County Adopt a Family 2025 Registration

This program is only available to families with children ages 0 to 18, located in Polk County, Oregon.

This program is intended to support families who are struggling to provide gifts for their children during the holiday season. Please tell us below if you are connected to any of the following programs.

• Oregon Health Plan (OHP)
• ODHS Food Benefits (SNAP)
• ODHS Cash Assistance (TANF)
• Employment Related Day Care
• Social Security Disability 

If you are not connected with any of the programs below, please provide a detailed explanation about the hardship your family is currently facing.
